Pipeline forecasts break because they're built on opinions. Not bad intentions, just opinions. And opinions don't compound into accuracy. They pile up into quarter-end surprises.
Here's the pattern. A rep marks a deal as "Proposal Sent" because they emailed a PDF. Did the prospect open it? Unknown. Did anyone on the buying committee review it? Unknown. - The prospect asked about implementation timelines

- No email reply in 14+ days
- Meeting rescheduled twice
- The contact who was engaged left the company
- No new stakeholders have appeared
- Proposal sent, zero follow-up activity
None of these require asking "how do you feel about this deal?" They're facts. They happened or they didn't.
A weekly forecast process that holds up
Here's the process I've seen work across teams of 5 to 30 reps. It takes about 45 minutes on Monday morning. It replaces the two-hour opinion-trading session most teams run.
Step 1: Sort by last activity date, not stage
Pull your pipeline. Ignore the stages. Sort every deal by the date of the last observable activity — last reply, last meeting, last click. Anything older than 10 business days goes into a "stalled" bucket automatically.
This single step usually removes 20-30% of the pipeline that was inflating the number.
Step 2: Apply the evidence test
For every deal still in the active bucket, ask one question: what happened this week that the prospect did, not what the rep did?
Rep sent a follow-up email? That's rep activity, not prospect activity. Prospect replied, clicked, or showed up? That's evidence.
Deals with zero prospect activity in the current week get flagged. Not removed — flagged. They're at risk.
Step 3: Weight by proof, not stage
Traditional forecasting weights deals by stage: "Discovery = 20%, Proposal = 60%, Verbal = 80%." This is meaningless because the stages are self-reported.

The difference: every level requires something the prospect did, not something the rep reported.
Step 4: Run the Monday number
Multiply each deal's value by its proof-based weight. Sum it up. That's your forecast.
Compare it to last Monday's number. Did the pipeline grow? Did deals advance (proof points added)? Did anything stall?
The trend line across four Mondays tells you more than any single snapshot.
Making it stick
This process fails when it becomes optional. Two patterns that keep it running:
Pattern 1: The forecast is the meeting. Don't do a pipeline review and then a forecast separately. The Monday session is the forecast. When the rep presents a deal, they present the evidence. No evidence, no pipeline credit. This takes about ten seconds per deal to establish as a norm.
Pattern 2: Stalls get addressed, not hidden. When a deal hits the stalled bucket, the conversation shifts from "what happened?" to "what's the next test?"
